There are libertarians with sincere belifs that I might disagree with but still respect. And then there are libertarians who seem to be little more than pseudo-intellectual trolls. Rand Paul is in the latter category.
If there was one form of disastrous pollution you would think everyone would readily agree the Environmental Protection Agency should regulate, it would be mountaintop removal coal mining. It’s hard to think of a modern industrial practice that does more damage than the one that blows up the top of mountains, fills up stream beds and valleys, and poisons the local water supply.

This is why Rand Paul is a joke. He has sincere, deeply held but not thought out beliefs. He is against socialism, except when it benefits him. He wants to cut spending, but not on spending that goes to his interests.

Gee, don’t they know libertarians are against initiation of force? But I’m sure there was an axiomatically correct reason to attack her.
Libertarian ideology rejects most of the modern regulatory systems that protect consumers, because everyone should be responsible for determining whether the hamburger contains E. coli on his own. Paul is obviously well within his rights to get mad about some dumb policy and write cranky letters about it and then form his own medical board with pizza parties every day and no girls allowed. It is just not clear why he was still claiming to be certified by the ABO years after he began his crusade against them.
